Our client was hurt in June, 2010 when he slipped and fell while trying to avoid being hit by a forklift. He has since undergone medical care in the physician’s office and Bolingbrook, Illinois.
Due to the accident, the claimant was off work for a period of time and on light duty for several weeks after that.
